Chuck E. Cheese In-Kind Donation Request

Please complete the form below to request an in-kind donation for your upcoming event. This form is intended for requests made on behalf of any non-federally tax-exempt organizations, which might include: 

• Neighborhood association

• Community group

• Informal organization

• Other non-exempt entity

Before you submit, please note: 

• Donation requests must be submitted at least 30 days before your event date. 

• If your event is fewer than 30 days away, please do not submit a request — we won't be able to process it in time.  

• Each organization is limited to one (1) in-kind donation request per 12-month period. If your group has received a donation from us within the past year, we ask that you check back with us at a later date.
